In order to expedite and facilitate our service of formal Notices and our voting process, we are collecting email addresses and formal consents to participate in electronic voting and service.
Your decision to participate in either electronic service of notices or electronic voting or both is completely voluntary. Your consent can be withdrawn at any time.
If you would like to participate in receiving electronic notices or voting electronically, please print and fill out this consent and deliver it to the CHA Office. Or you can pick up a hard copy at the CHA office. CONSENT FORM
We will begin this modernization once we receive sufficient consents and the necessary software.
